Situated Learning Strategy for General Comprehensive Medicine Specialists' Performance in Cardiovascular Risk Management

Abstract

Introduction: Cardiovascular diseases are the leading cause of global mortality. In Cuban primary healthcare, gaps exist between the theoretical preventive model and reactive practices regarding cardiovascular risk stratification.

Objective: To evaluate the effectiveness of a training strategy based on situated learning for improving the professional performance of General Comprehensive Medicine Specialists in cardiovascular risk management at the "Ramón López Peña" Polyclinic during 2024.

Methods: An educational health intervention study was conducted using a pre-experimental design (one-group pretest-postest). From a population of 116 specialists, an intentional sample of 27 doctors was selected. The Objective Positioning Index (OPI) was assessed through observation guides, medical record audits, and dimensional clinical performance tests. Data were analyzed using the Wilcoxon signed-rank test.

Results: The group's overall OPI significantly increased from 1.40 points (low level) in the pretest to 3.90 points (medium level) in the postest, representing a net gain of 2.50 points. Although the technological-digital and ethical-communicative dimensions showed a notable qualitative impact, none reached the high-level threshold due to institutional time constraints. The Wilcoxon test demonstrated that this change in overall performance was highly significant (W = 378.0); (p < 0.001).

Conclusions: The situated learning strategy is methodologically effective. It successfully shifted professional performance from a low to a stable medium level, and encouraged a predictive, digitally mediated, and deliberative praxis through the use of mobile health; however, its definitive consolidation requires long-term, sustained institutional support.
