Frequency of Clinical and Paraclinical Aspects in Deaths from COVID-19 in a Quito Hospital

Abstract

Introduction: COVID-19 has had a significant impact on global health, causing the death of millions of people due to various associated complications. Its clinical evolution remains unpredictable, so it is essential to analyze the characteristics of those who have experienced serious complications or died. This will help improve predictive models and optimize the response to future health crises.

Objective: Identify the clinical, paraclinical characteristics and comorbidities of deceased patients infected by SARS-CoV-2 treated at the General Hospital of the South of Quito.

Methods: A retrospective, cross-sectional and descriptive cohort study was carried out during the period March-November 2020, at the General Hospital of the South of Quito of the Ecuadorian Institute of Social Security. Percentages of categorical variables and means and standard deviations of continuous variables were calculated.

Results: A total of 95 consecutively admitted and deceased patients were found, 66.3% were men and 65.3% were adults ≥65 years of age. The most frequent symptoms were: dyspnea (73.7%), dry cough (51.6%) and temperature rise (46.3%). The most frequent comorbidities were: arterial hypertension (33.7%) and diabetes mellitus (22.1%). 56.3% had CO-RADS lung tomographic score 5, and the NEWS-2 scale score was ≥7 points in 23.2%.

Conclusions: It was identified in 95 patients who died from COVID-19, aged ≥65 years, that the highest risk factors were being male, having type 2 diabetes mellitus and/or high blood pressure, as well as presenting symptoms such as dyspnea, dry cough and fever. The most common clinical signs observed on physical examination were tachypnea, tachycardia, and severe hypoxia. In complementary examinations, greater pulmonary involvement was highlighted (CO-RADS ≥5). The average hospital stay was 8.0 days.
