Etiological, Diagnostic and Therapeutic Approach to Diabetes Insipidus from the First Level of Medical Care

Abstract

Diabetes insipidus is an illness characterized by the inability to concentrate urine. Polyuria and polydipsia are the predominant clinical expressions. This paper examines the diagnostic and therapeutic approach when this disease is suspected. A review of national and international literature from the last 12 years was carried out in PubMed, Medline, Scopus, SciELO, Elsevier, JAMA, other open access journals, and the Google Scholar search engine. The keywords "diabetes insipidus", "polyuria", and "hypernatremia" were used, along with their Spanish equivalents, retrieving documents in both languages. The quality, reliability, and validity of 37 selected articles were analyzed; 20 of them were referenced. Etiological, diagnostic, and treatment aspects were reviewed, together with some considerations on the diagnostic approach. Diabetes insipidus is a water metabolism disorder that presents with polyuria and polydipsia. It is classified as central (CDI) or nephrogenic (NDI). Both forms can have genetic or acquired causes. Diagnosis is confirmed by low-density polyuria and the water deprivation test. The mainstay of CDI treatment is desmopressin in its different formulations.
